The Benefit Choice Fair provides the opportunity to speak with representatives from Human Resources and from the
various benefit plans offered to Illinois State University faculty and staff. The following are some of the changes you may
make during Benefit Choice Periods.
Click here for a list of forms to do any of the following:
• Add or drop dependents from your group insurance
• Change health plan providers
• Enroll in the Flexible Spending Programs (MCAP or DCAP)
• Make changes to your tax deferred annuity
• Opt in or out of the health, dental and vision coverage
• Update your beneficiaries
• Enroll unrelated same-sex Domestic Partner, including those previously terminated for non-payment of
premium (complete Domestic Partner Enrollment Packet available online at www.benefitschoice.il.gov)
