Health Insurance

Employees may choose the Quality Care Health Plan or a Managed Care Health Plan. Quality Care has an annual deductible & copayments, after which most covered expenses are reimbursed at 80 percent of ususal and customary charges. Under the two managed care HMO plans, there are nominal charges for doctor’s visits and prescription drugs. Most other expenses are paid at 100 percent. The third managed care plan has three tiers of coverage ranging from an HMO, to a PPO which pays 90/10, to a quality care plan. All plans have a monthly employee premium based on salary and an additional monthly premium for dependent coverage. Also, you may opt out of insurance coverage by submitting required documentation. Vision coverage is provided through EyeMed Vision Care.
