Applicant or Employee Accommodations
An applicant or employee who believes he/she is an individual with a disability and needs a reasonable accommodation under the Americans with Disabilities Act (ADA) or other state or federal law may request accommodation by contacting the Employment Accommodations Coordinator in Human Resources. Accommodation decisions are made in accordance with University policy.
As part of the Reasonable Accommodations Procedure, employees and applicants will need to complete the Employment Accommodation Request form and the Medical Information Request form.
Any medical documentation submitted to the Office of Human Resources is confidential and will be filed separately from applicant records or employee personnel files.
