If you were previously a student at Illinois State University or the Laboratory Schools, you will need to change your name through the Student Service & Referral Center (SSRC).
To complete the name change, you must provide a copy of your marriage license, a court order, or a dissolution decree certifying the name change. Send a photocopy of one of these documents, a copy of a photo ID and a written or faxed request. Be sure to sign this request as without your signature, your name change cannot be processed.
For further information regarding this process, contact the SSRC at 309-438-2188 during normal business hours (8am to 4:30pm Monday through Friday).

To complete the name change, you must provide a copy of your marriage license, a court order, or a dissolution decree certifying the name change. Send a photocopy of one of these documents, a copy of a photo ID and a written or faxed request that includes your University ID (UID). Be sure to sign this request as without your signature, your name change cannot be processed. You may also elect to bring the appropriate documentation and request into the Human Resources office.

Once you have completed the process to change your name, follow these steps to have your ULID updated.
Send an e-mail to Randy Marrs at rmarrs@ilstu.edu and provide the following information:
Indicate in the e-mail that you would like to have your ULID changed to reflect your new name. Randy will then respond to you with your new ULID.
Once you have completed the process to change your name, follow these steps to have your NVAS User ID changed.
Send an e-mail to John Clement at jmcleme@ilstu.edu and provide the following information:
Indicate in the e-mail that you would like to have your NVAS User ID changed to reflect your new name. John will then respond to you with your new NVAS User ID.
Note: The NVAS User ID is used to log on to the Mainframe.
