Non-Tenure Track Faculty Hiring Forms

Listed below are the forms required to hire a new NTT faculty member:

• PERS 900–Personnel Action Form (PAF) (To request, e-mail:
• PERS 150–Vita Sheet
• Employment Eligibility Verification–Form I-9 (Not applicable to Adjunct faculty)
• Official transcripts
• PERS 931–Affirmative Action Information Search Form* (Not applicable to Adjunct faculty)

• SSA-1945–Statement Concerning Your Employment in a Job Not Covered by Social Security (Not applicable to Emeriti or Adjunct faculty)
• PERS 903– Address Information Form

*If an appointment is unique or short term in nature, a PERS 930 Request for Waiver of Affirmative Action Search procedures may be requested from the Office for Diversity and Affirmative Action, and, if granted, filed with the appointment papers instead of the Search Form.
