Necessary Documentation

  • PERS 900 - PAF - or PERS 910 if previously hired
  • PERS 931 - Affirmative Action Information Search Form
  • PERS 150 - Vita Sheet (not necessary if current PERS 150 is on file in the Office of
    Human Resources)
  • PERS 916 - Additional Payment (only if appointment is in addition to 100 percent A/P or Civil Service assignment)
Official transcript (if not on file in Office of Human Resources)
  • PERS 903 - Address Information Form - to be sent directly to the Office of Human Resources, mail code 1300
  • Verification of Oral English Language Proficiency (if applicable) (box
    provided on PAF)
  • Employment Eligibility Verification (Form I-9) (if a current form is not on file) (not necessary for Adjunct appointments)

If an appointment is very unique or short term in nature, a PERS 930 Request for Waiver of Affirmative Action Search procedures may be requested from the Affirmative Action Office, and, if granted, filed with the appointment papers instead of the Search Form.

Each initial and succeeding PAF must be signed by the Departmental Chairperson, the College Dean and, when applicable, the Other Fund Coordinator before being forwarded to the Office of Human Resources for final approval and data entry. The Office of Human Resources will then receive a system-printed turnaround PAF and employee contract, when required, and will distribute these forms as applicable.

A PAF will be issued automatically before the beginning of each academic year for those faculty on a tenured/tenure-track appointment or on an adjunct appointment. Departments need only initiate a PAF on tenured/tenure-track Faculty in cases where it is necessary to update data - specifically in the areas of personal data and funding information. Data concerning Leaves of Absence and Promotions and Tenure granted through the ASPT process will be updated automatically by the Office of Human Resources and a revised PAF will be received by the Employing Department. For Faculty on nontenure-track appointments a PAF must be initiated by the department for each successive contractual period. No Faculty hiring may be completed unless a position exists which will accommodate the individual being hired.
