• PERS 903 - Address Information Form - to be sent directly to the Office of Human Resources, mail code 1300
• PERS 919 - Request for Tenure Year Designation
• Verification of Oral English Language Proficiency (if applicable) (box provided on PAF)
• Employment Eligibility Verification (Form I-9) (if a current form is not on  file)
