- Set goals and objectives for the next evaluation period.
Procedure:
- Complete an evaluation instrument,
- Complete the Performance evaluation Summary Form and attach it to the front of the written evaluation,
- Discuss the evaluation with the employee,
- Allow the employee an opportunity to respond to the evaluation,
- Acquire signatures from employee and supervisor,
- Send Performance evaluation Summary Form and evaluation to the Office of Human Resources (Mail Code 1300)
The original copy of each completed evaluation is to be returned to the Office of Human Resources (Mail Code 1300 or 101 Nelson Smith Building) no later than April 1, of the current fiscal year.
A number of Administrative/Professional employees also hold academic rank. University policy requires that their A/P positions be evaluated even though they may be subject to an academic evaluation. For Administrative/Professional employees who work for two departments, evaluations are needed from each department.
