Completed Evaluations are due no later than April 1
Performance evaluation Summary Form
The original copy of each completed evaluation is to be returned to the Office of Human Resources (Mail Code 1300 or 101 Nelson Smith Building) no later than April 1, of the current fiscal year.
A number of Administrative/Professional employees also hold academic rank. University policy requires that their A/P positions be evaluated even though they may be subject to an academic evaluation. For Administrative/Professional employees who work for two departments, evaluations are needed from each department.
