The performance appraisal process serves two main purposes.
Performance Appraisals are required for Civil Service employees on an annual basis. Additional appraisals are required during any probationary period.
The Office of Human Resources provides a Standard Evaluation Form for both exempt and non-exempt Civil Service positions. We recognize that different areas may benefit from customized forms. Units and departments are encouraged to develop their own forms to better suit their specific needs. Forms must be reviewed by Human Resources prior to use.
