Lab School Faculty

Teachers in the Laboratory Schools are classified as Faculty Associates.  Faculty Associates are assigned to one of the University Laboratory Schools. These appointments may be on either a Continuing or Non-continuing Appointment Type, depending upon the qualifications of the position incumbent and the allocation of the position.

Paperwork to hire a Faculty Associate Through the Online System:

• Selection Form (Online Employment System)
• Vita Sheet
• Official transcripts
• Illinois Teaching Certificate
• Campus Security Forms (Online Employment System)
• New Employee Address Information Form
• Employment Eligibility Verification (Form I-9)
• Offer Letter
• TB Test Results (must be completed within 90 days of begin date)
• CANTS 22 DCFS Mandated Reporter form
• SSA-1945 Statement Concerning Your Employment in a Job Not Covered by Social Security

Paperwork to hire a Non-Continuing Faculty Associate Outside of the Online System:

• Personnel Action Form (PERS 900)
• Vita Sheet
• Official transcripts
• Affirmative Action Waiver from Search *
• Illinois Teaching Certificate
• New Employee Address Information Form
• Employment Eligibility Verification (Form I-9)
• Offer Letter
• Campus Security Act Conviction Information Request form
• Fingerprinting memo
• TB Test Results (must be completed within 90 days of begin date)
• CANTS 22 DCFS Mandated Reporter form
• SSA-1945 Statement Concerning Your Employment in a Job Not Covered by Social Security

*If any appointment is very unique or short term in nature, a PERS 930 Request for Waiver of Affirmative Action Search procedures may be requested from the Office of Diversity and Affirmative Action, and, if granted, filed with the appointment papers instead of the Search Form.
