Family Medical Leave Act (FMLA)

Employees of Illinois State University may take up to 12 weeks of paid/unpaid, job-protected leave for certain family and medical reasons. All approved requests for Extended Illness Leave will count toward the 12-week limit if the employee qualifies for an FMLA leave. The employee must have worked at Illinois State University for at least one year and worked at least 1250 hours during the previous 12 months.

Employer Responsibilities  

  • Initiate the FMLA process if aware in advance of a eligible employee with a valid reason for leave.
  • Notify HR when an employee calls in after more than three consecutive days off
  • Notify HR when an employee is absent for more than three consecutive days.
  • Complete and return the record of leave form to Benefit Services.
