Workers' Compensation
If you have suffered a work-related injury and want to file a Workers' Compensation claim, please contact the Office of Human Resources at (309) 438-8311.
Supervisors, Forepersons, or other appointed and authorized individuals are required to file a workplace injury report on the "Injury Log" at the Environmental Health and Safety website. The Injury Log provides all necessary forms as well as contact information for the University Preferred Provider for medical treatment of work-related injuries.
