All participants are required to have the followingforms on file in the Wellness office prior to participation. Health forms need to be updated every three years upon initial completion, or more frequently if your health status changes. If your health status changes, please inform the Wellness Program Director in writing.
For Illinois State faculty and staff who wish to have their payments payroll deducted, please use the Wellness Payroll Deduction Form included on the bottom of the registration form. Checks may be written to ISU Wellness Program.
The Wellness Program's primary purpose is to serve the employees, retirees and adult family members of Illinois State University.
